  1. Fill out the enclosed form.
  2. Parent/Guardian must sign at BOTH X’s and INITIAL the Photo Disclaimer box
  3. Mail both copies of the form, copy of birth certificate and your check payable to:

Registrations for all areas will take place at: CHAMPIONS FOR LIFE SPORTS CENTER (453 Grant Avenue, Auburn, N.Y.)

  • Saturday, February 26th from 9 am—11 am
  • Saturday, March 5th from 9 am—11 am
  • Wednesday, March 9th from 6—7 pm
  • Saturday, March 12th from 9 am—11 am
  • Wednesday, March 16th from 6—7 pm
  • Saturday, March 19th from 9 am—11 am
  • Wednesday, March 23rd from 6 –7 pm
  • Saturday, March 26th from 9 am—11 am
